                                                                    Have all documents, photo, and birth certificate ready before submitting form
FIRST: Print & Complete both waiver forms below. Be sure to hand-sign and date each form.
Then scan or take a high-quality photo of each and attach each document into the appropriate place below. 

1. US CLUB Waiver Form
2. PISA WAIVER FORM 

SECOND:  Attach a player head-shot photo (less than 1MB please)
THIRD: Attach copy of birth certificate (not required for returning Hurricanes players).
